Sarah Lenker
Occupational Safety Investigation SME
SMSgt (Ret.) Sarah Lenker serves as an Occupational Safety Advisor for ANG/SE, where she supports all ANG occupational investigations and provides daily guidance to wing-level safety personnel. Her support spans mishap investigation, corrective action and recommendation management, AFSAS oversight, and overall occupational safety program execution.
Prior to joining ANG/SE, Sarah served 11 years on active duty as an Occupational Safety professional, with assignments at four installations and one deployed location. She spent two years at the Air Force Safety Center in the Occupational Safety Division’s Reports, Investigations, and Analysis Branch, supporting all on-duty Class A and B Safety Investigation Boards. In that role, she advised investigators, conducted report reviews, and performed trend analysis to inform prevention strategies and policy improvements.
Sarah also led the Safety Academy at JBSA-Lackland for three years, overseeing the development and instruction of three Occupational Safety courses and one Weapons Safety Managers course. She has been with Creek Technologies since November 2024.
David Moore
Aviation Safety Investigation SME
Lieutenant Colonel (Ret.) Dave Moore concluded a 31-year Air Force career as Chief of Wing Safety for the 439th Airlift Wing at Westover Air Reserve Base, a major C-5 Galaxy installation. During his final seven years on active duty, he led the wing’s safety program, overseeing aviation risk management, mishap prevention, and regulatory compliance across complex airlift operations.
After retirement, he served eleven years as a civilian Flight Safety Assistance Advisor to Air Force Reserve Command, supporting Reserve Safety Officers and NCOs in strengthening their aviation safety programs. Since 2015, he has served as Aviation Safety Advisor to the Air National Guard, assisting Chiefs of Safety, Flight Safety Officers, and Flight Safety NCOs with investigations, program execution, and policy implementation. He is a graduate of the Flight Safety Officer Course, Chief of Safety Course, and Operational Risk Management Applications and Integration Course, and remains a long-time instructor in the Air Reserve Component Chief of Safety Course.
As a Flight Examiner, Instructor Pilot, and Aircraft Commander, Dave accumulated more than 7,500 mishap-free flying hours in the C-5A/B, C-130E, C-123A, and T-29A/B/C. He also served two terms on the Bird Strike Committee USA and presented at the North American Bird Strike Conference on wildlife risk management at Westover Air Reserve Base.
Jeffrey Thomas
Aviation Safety Investigation SME
Jeff Thomas served as a Wing Flight Safety Officer in both Air Education and Training Command and Air Mobility Command before being selected as Chief of Flight Safety for Pacific Air Forces. In these senior leadership roles, he provided enterprise-level oversight of aviation safety programs across multiple wings and major commands, ensuring effective implementation of Air Force safety policy and mishap prevention initiatives. He later served at Headquarters Air Force Safety Center as Aviation Investigations Team Chief, leading and coordinating high-visibility aircraft mishap investigations and supporting field safety organizations worldwide.
Over the course of his career, Jeff served as a board member on more than 30 Class A flight mishap investigations for the United States Air Force and several international partner air forces. He brought deep expertise in investigative methodology, analysis, and report development, shaping findings and recommendations that informed policy and operational risk reduction across the enterprise.
Jeff was also a distinguished instructor in advanced safety courses, including the Aircraft Mishap Investigation Course, the Board Presidents Course, and the International Flight Safety Officer Course, where he trained Flight Safety Officers, senior leaders, and international partners in investigative techniques and accident prevention strategies. In recognition of his sustained impact on aviation safety, he was inducted into the Air Force Safety Hall of Fame in 2007.
Jennifer Logsdon
Aviation Safety Investigation SME
Jen is a KC-135 pilot and Squadron Assigned Flight Safety Officer with the 127th Wing at Selfridge ANGB, Michigan, where she currently serves as a drill status guardsman. She previously served as the wing Flight Safety Officer and has been a board member on a Class A Safety Investigation Board. Prior to commissioning, she served as an F-16 egress mechanic with the 180th Fighter Wing in Toledo, Ohio, bringing both maintenance and operational perspectives to aviation safety.
Since 2022, Jen has supported ANG/SE as an Aviation Safety Advisor, teaching multiple sections of the Air Reserve Component Safety Course each year. In this role, she provides direct support to ANG Class B Safety Investigation Boards and delivers daily guidance to wing-level safety personnel in mishap investigation, corrective action management, and AFSAS execution.
